To be honest I really don't know. I've always restarted IPtables out habit. Even if it automatically scans the rules I want to be sure my changes are installed. It takes all of 2 seconds to restart IPtables so I'd suggest restarting IPtables period. If you are making a firewall change you have a reason for doing so and want to test it right way. So why chance that it automatically be re-read? If there is an error in my edits I want to see it right away and know about it right away.

It depends on what you mean by changed the rules. If you entered iptables commands to set rules then they're already active. If you set them in a config file somewhere, then you probably have to restart the service to apply them.
