The Modem plays no roll in this. This is a modem/router combo so lets ignore semantics. The router is only involved because it is the thing your connecting to. But what we need to know is the model of the extender. This way we can find the specific instructions for it. Because, I don't know the specific model I can only be vague. The basic idea is you are going to connect to the extender. From there you will enter your wifi router's network name (SSID) and password into the extender's settings. You do not need the IP address as the router will hand that out automatically once connected.

So, sounds like you know how to find the info you need. (To many people don't). you were just looking at the wrong device. Look for the manual for the extender, it will tell you exactly what to do.

Correction. After typing this whole thing I realized that you did put the model in the question. So yes, I am one of those people that don't read that I was just insulting.

Search for "NETGEAR AC750 WiF Range Extender Model EX3700 User Manual" (Yahoo will hide my answer if I post the link). YOu will see a link to a pdf on Netgear's site. The instructions you need start on page 10. You will use use the browser to connect and skip the WPS portion. Your router doesn't appear to have a WPS button, so that method would be much more complicated than the other set of directions.
